The clinical pharmacy coordinator is responsible for providing pharmaceutical care with a specific focus on cell and gene therapy to patients managed in both our inpatient and ambulatory clinics at the Hospital of the University of Pennsylvania. The specialist will be expected to act as a liaison to the Cell Therapy Team and both assist in coordinating the onboarding of new therapies, provide clinical management of our ambulatory patients. Activities include evaluating, monitoring and modifying drug/cell therapy, providing recommendations to optimize patient outcomes, providing patients with new medication education, minimizing safety issues and ensuring cost effectiveness, and providing drug information for medical personnel. The pharmacy coordinator will provide subject matter expertise for the provision of care through EHR builds and clinical operational management. The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the pharmacy department. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting. The pharmacist will also assist with collection and presentation of patient outcomes data and quality improvement projects and align with the efforts of our Cell Therapy Team. The clinical pharmacy specialist will be expected to participate in the education and training of students, PGY1 and PGY2 Oncology residents. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO and other regulatory agencies.
