The University of Iowa Health Care Department of Neurology is seeking a Social Worker III to support the Parkinson's Center of Excellence. This position will provide the social work services (e.g., preparing psychosocial histories, recommending and planning social treatment methods, organizing support group meetings and patient symposia, etc.) in the coordination of care for clinic patients diagnosed with Parkinson's Disease (PD). The Social Worker will also serve as a liaison to the Parkinson's Foundation as the Parkinson's Center of Excellence (COE) Coordinator. The COE Coordinator is responsible for educating patients and families on the Foundation's resources, ensuring programs are active and running smoothly, and best practice guidelines are properly implemented at the Center.
Job Duties to Include:
- Prepare psychosocial histories, participate in clinical staff diagnostic and treatment staffing, interpret patients' and clients' and their families' psychosocial functioning and needs, and recommend and plan social treatment methods to meet these needs
- Plan for and provide liaison, coordination and referral services regarding patients and clients and their families
- Provide direct services to include casework, social group work and family therapy as assessed and recommended
- Provide indirect service to patients and clients through responsibility for and participation in program development, teaching and research
- Plan for and provide consultation to medical or clinical service staff and community social agencies regarding the handling of medical-social or other social functioning problems of individual of groups of patients and clients and their families
- Contribute to the formation of department, hospital or clinic policies affecting patient and client care, training programs and departmental and professional growth
- Participate in departmental or clinic staff meetings and planning sessions and contribute to the solution of department of clinic management problems
- Participate in related research projects. Participate in the development, strengthening and delivery of needed health and social services
- Write case reports covering ongoing social services for inclusion in the patients' or clients' clinical records, maintain statistical records of social service activity
- Facilitate communication between patients and members of the Parkinson's Disease (PD) care team, including alerting the team about psychological, social, and financial concerns, as well as developing a plan for meeting these needs
- Assist patients and families navigate a PD treatment plan
- Educate patients and their family, the community, and other relevant healthcare professionals about the Parkinson's Foundation programs and resources, the COE's work, and PD care. Ensure materials are readily accessible
- Coordinate the planning and execution of outreach programs and events
- Participate in Parkinson's meetings (i.e., taking and sharing information, acting as a resource/presenter/advisor throughout the COE network, etc.)
- Provide leadership opportunities, education, and recognition within the COE
- Work with Medical Director to ensure the COE maintains its designation as Center of Excellence
- Attend and participate in courses, meetings, conferences and workshops in attainment of new or advanced social work practice skills
- Maintain effective working relationships with faculty, staff, students and the public

Required Qualifications
- Master's Degree with a major concentration in social work
- Some (6 months - 1 year) social work experience
- A license to practice social work in Iowa
- Excellent written and verbal communication skills
- Experience using Epic or electronic health record (EHR) systems
Desired Qualifications
- Social work experience in an academic health care setting with an emphasis on social work assessment and discharge planning with primarily adult patients and their families
Case management or care coordination experience
Ability to manage multiple tasks at once, to maintain positive relationships with team members and the ability to understand, interpret and respond appropriately to the emotions of patients, families, and peers
Flexibility to collaborate as part of multidisciplinary care teams, as well as good problem-solving skills, including ability to effect change and resolve calcitrant problems
Experience with financial resources, including insurance coverage options, changes related to the Affordable Care Act, and the disability process, as well as community resources, home healthcare, and nursing facility placements available for patients in outpatient settings
Experience working as part of a multidisciplinary team and/or clinic, specifically a Movement Disorders or Parkinson's Disease clinic
Some familiarity with social work undergraduate education
Demonstrated interest in working with patients who have neurogenetic or neuromuscular diseases
Knowledge of UI policies and procedures
